Iron for children aged 1 to 3

Seven milligrams a day, and one of the most-missed targets in early childhood. Toddlers are the classic iron-deficiency group in Europe, and the usual culprit is not too little food but too much cow milk.

A small stomach and a fast-growing body

EFSA sets 7 mg a day for children aged 1 to 6. In the first years that is a demanding figure relative to how much a toddler actually eats: blood volume and lean mass are expanding quickly, the iron endowment from birth is used up during the first year, and the total amount of food going in is small.

Iron deficiency in this age group is not a curiosity. It is the commonest nutritional deficiency of early childhood in Europe, and it matters because the deficiency period coincides with rapid brain development.

The cow milk trap

The single most common cause is a large intake of unmodified cow milk. Cow milk is very low in iron, its calcium and casein interfere with the absorption of iron from other foods, and a child drinking a lot of it is full and eating less of everything else. In some children it also causes small chronic gut blood loss.

The practical answer is not to remove milk but to cap it, and to keep it out of the position where it displaces meals. National child health guidance in the Nordic and Baltic countries gives specific volume limits for this age; a family health nurse or paediatrician is the right place to get the local figure.

What to put on the plate instead

Meat and fish in small amounts are the most efficient contributors because their iron is haem iron. Beyond that: lentils and beans mashed into familiar dishes, wholegrain rye bread, oats, egg, and vegetables and berries with vitamin C alongside them to lift absorption from the plant sources.

Iron drops and syrups are prescription-grade decisions, not shelf ones. Iron is the leading cause of serious accidental poisoning in small children, and any iron product in the house should be treated with the same care as medicine.

When to ask

Pallor, unusual tiredness or irritability, poor appetite, or eating non-food items such as soil or paper are reasons to see a doctor. So is a diet that has settled into milk and a very narrow range of foods. A blood test answers the question that a table cannot.
